Registering with the surgery
To register with the surgery, please choose one of the following options:
- Complete a new patient registration form online:
- Visit reception from 10am to 6pm and collect a paper registration form.
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
We ask that new patients complete our registration form, rather than the generic NHS form, because it allows us collect and to enter onto our system all the information we need to offer you the best standard of care.
For example, whether you have ever served in the armed forces (or are related to someone who has), or whether you need any reasonable adjustments making to help you access care.

Temporary Residents
Temporary registration is available for shorter periods if you are away from home and need medical care. You will be asked to fill in a temporary patient registration form.
