Submit your blood pressure readings

Use this service to submit your blood pressure readings to the surgery.

The British Heart Foundation has tips about how you can manage your blood pressure and health.

You can use this service if you:

  • are registered at the surgery
  • have been invited to do so

Before you start

We’ll ask you for:

  • your first and last name, date of birth, sex, postcode, email and phone number
  • if applicable, the details of the person you are completing the form on behalf of
  • up to 7 days of blood pressure readings
  • your smoking status
  • your weight

We suggest that all our patients buy a suitable blood pressure machine and check how to take their blood pressure. We advise a machine with an upper arm cuff.

If you are a patient with high blood pressure, you will need to provide us with 7 days of readings every 6 months. You will also need to record your recent alcohol consumption, smoking status and weight at the same time.

We may need you to have a blood test annually or every 6 months for some medications, for which you would need to book in to one of our phlebotomy clinics.
