New patient registration (over 16)

Use this service to register with the surgery.

You can use this service if you:

We will always try to:

  • give you an appointment on the day, if you need an urgent consultation
  • see you within 20 minutes of your booked time or advise you if a clinician is running late or there is an emergency situation in progress
  • we will keep the contents of your medical record confidential
  • should you need to complain about our services, we will respond according to NHS complaints procedure guidelines
  • when you make a suggestion or comment on our services, we will welcome your thoughts and respond promptly
  • we will be courteous and respectful at all times

Please could we ask that:

  • you always attend your appointments punctually, or let us know if you are unable to attend
  • you are prepared to tell the Receptionists a few details about a medical problem – we have many other clinicians working with us, who it may be better to direct you to, if we have enough information
  • you are aware that our resources are finite and use them responsibly
  • when you change your name, address or telephone number you will inform us immediately.
  • you please only request a home visit if it is impossible for you to get to the surgery
  • following a consultation, please act upon the advice and take medication as prescribed. You too are responsible for your health
  • when dealing with all members of the practice, please treat them as you would wish to be treated

We ask that you provide one form of identification for online services (which detail your address and photograph).

We also offer a New Patient Health Check to all newly registered patients. Your records may take several weeks to reach us, and this Health Check allows us to start to build a picture of your health needs. Under 5-year-olds are looked after in conjunction with the Health Visiting Team.

Before you start

We’ll ask you for:

  • your first and last name, date of birth, sex, postcode, email and phone number
  • if applicable, the details of the person you are completing the form on behalf of
  • your NHS number
  • basic health and medical information
  • other personal details and demographics
